Medical Use
Epidiolex is prescribed for managing seizures in patients aged 12 months and older who have Lennox-Gastaut syndrome, Dravet syndrome, or Tuberous sclerosis complex.
Recommended Dosage:
- -Before Treatment: Check serum transaminases (ALT and AST) and total bilirubin levels.
For Seizures Associated with Lennox-Gastaut Syndrome or Dravet Syndrome:
- -Initial Dose: 2.5 mg/kg orally twice daily (5 mg/kg/day).
- -After 7 Days: Increase to a maintenance dose of 5 mg/kg twice daily (10 mg/kg/day).
- -Maximum Dose: Can be increased up to 10 mg/kg twice daily (20 mg/kg/day) based on clinical response and tolerability.
For Seizures Associated with Tuberous Sclerosis Complex (TSC):
- -Initial Dose: 2.5 mg/kg orally twice daily (5 mg/kg/day).
- -Weekly Increments: Increase by 2.5 mg/kg twice daily (5 mg/kg/day) to a maintenance dose of 12.5 mg/kg twice daily (25 mg/kg/day), as tolerated.
- -Rapid Titration: If needed, increase dosage no more frequently than every other day to reach 25 mg/kg/day.
Note: The effectiveness of doses below 12.5 mg/kg twice daily has not been evaluated for TSC.
Patients with Hepatic Impairment:
- -Dose Adjustment: Required for moderate (Child-Pugh B) or severe (Child-Pugh C) hepatic impairment.
Warning & Precautions
Epidiolex 100mg/mL may cause hypersensitivity reactions. If such reactions occur, discontinue the medication immediately. It should not be used in patients with a known hypersensitivity to cannabidiol or any other ingredients in the product.
Liver Transaminase Elevations: Cannabidiol can cause dose-related increases in liver enzymes (ALT and AST). Before starting treatment, check serum transaminases (ALT/AST) and total bilirubin levels.
Somnolence and Sedation: Cannabidiol may cause drowsiness and sedation. Patients should be monitored for these effects and advised not to drive or operate machinery until they are familiar with how the medication affects them.
Suicidal Thoughts and Behavior: Antiepileptic drugs (AEDs), including Cannabidiol 100mg/mL, can increase the risk of suicidal thoughts or behavior. Patients should be monitored for signs of depression, suicidal thoughts or behavior, or any unusual changes in mood or behavior.
Withdrawal: Cannabidiol should generally be tapered off gradually to avoid increased seizure frequency and status epilepticus. However, in cases of serious adverse events, rapid discontinuation may be necessary.
Pregnancy and Breastfeeding: The safety of Epidiolex during pregnancy and breastfeeding is not established. If you are pregnant, planning to become pregnant, or breastfeeding, discuss the potential benefits and risks with your doctor.
