Medical uses:

Livmarli is the only FDA-approved medication and an ileal bile acid transporter (IBAT) inhibitor commonly used to treat cholestatic pruritus (itching) in patients with Alagille syndrome (ALGS) who are 12 months and older.

Recommended Dosage: If you are taking bile acid-binding resins, Livmarli should be taken at least four hours before or after the resin. The recommended dosage is 380 mcg per kg once daily, taken 30 minutes before the first meal of the day. Start with 190 mcg per kg taken orally once daily; after one week, increase to 380 mcg per kg once daily, as tolerated. For those weighing over 70 kg, the maximum daily dose is 3 mL or 28.5 mg.

If a dose is vomited or missed, it should be taken as soon as possible within 12 hours of the usual time. If more than 12 hours have passed, skip the missed dose and continue with the regular dosing schedule.

Warnings & Precautions:

Baseline liver tests should be conducted, and patients must be monitored throughout treatment. If abnormalities arise without other causes, consider reducing the dose or pausing treatment. Discontinue treatment for persistent or recurrent liver test abnormalities.

Livmarli has not been evaluated in ALGS patients with cirrhosis. Monitor patients on Livmarli 9.5 mg oral solution for liver test elevations and liver-related toxicity.

Common side effects in patients treated with Maralixibat include vomiting, diarrhea, and abdominal pain. If these occur, consider dose reduction or interruption. Monitor for dehydration and treat promptly if vomiting or diarrhea occurs.

ALGS patients may have a deficiency in fat-soluble vitamins, which this medication can affect. Obtain serum FSV levels and monitor throughout therapy, along with any clinical symptoms. If a deficiency is detected, supplement with fat-soluble vitamins. If the deficiency persists or worsens despite supplementation, consider discontinuing Livmarli.
