Medical Use
Stem Cell Transplant Preparation in Children: Tepadina is used in children with severe beta-thalassemia to prepare for a stem cell transplant. When combined with high doses of busulfan and cyclophosphamide, it helps reduce the risk of transplant rejection.
Breast and Ovarian Cancer Treatment: This drug treats adenocarcinoma in the breast and ovaries, slowing or stopping cancer cell growth.
Managing Cancer-Related Fluid Buildup: For cancers that cause fluid accumulation in areas like the lungs or abdomen, Tepadina can be administered directly into the affected cavities to control the fluid and improve comfort.
Bladder Cancer Treatment: Thiotepa treats superficial papillary carcinoma of the bladder by being applied directly to the bladder, targeting cancer cells on the surface and preventing their spread.
Recommended Dosages
Stem Cell Transplant Preparation in Children:
- -Dose: 5 mg/kg intravenously, twice, 12 hours apart, on Day -6 before allogeneic HSCT, along with high-dose busulfan and cyclophosphamide.
Adenocarcinoma of the Breast or Ovary:
- -Dose: 0.3 to 0.4 mg/kg intravenously every 1 to 4 weeks. Start with the higher dose and adjust weekly based on blood counts, ensuring doses are not given more than once a week.
Malignant Effusions:
- -Dose: 0.6 to 0.8 mg/kg intracavitary, through the same tubing used to remove the fluid, every 1 to 4 weeks. Start with the higher dose and adjust weekly based on blood counts. Maintenance doses should not be given more than once a week.
Superficial Papillary Carcinoma of the Urinary Bladder:
- -Dose: 60 mg in 30 to 60 mL of Sodium Chloride, administered via catheter into the bladder. The solution should be retained for 2 hours. If 60 mL is difficult to retain, use 30 mL. Reposition the patient every 15 minutes for better contact. Treatment is typically once a week for 4 weeks. If needed, the course may be repeated, but with caution due to the risk of increased bone marrow depression.
Warning & Precautions
Administering Thiotepa injections (15 mg or 100 mg) can significantly reduce blood cell counts, heightening the risk of infections, bleeding, and anemia. It’s crucial to have regular blood tests to monitor these levels and address any issues promptly.
Some individuals may experience allergic reactions to Thiotepa. Be vigilant for symptoms such as rash, itching, or difficulty breathing. If these occur, discontinue the medication and seek immediate medical attention.
This medication can also cause skin reactions like redness, rash, or peeling. Notify your healthcare provider if you observe any unusual skin changes during treatment.
Avoid live or attenuated vaccines while on Tepadina (15 mg/100 mg), as it can weaken your immune system and diminish vaccine effectiveness. Always consult your healthcare provider before getting vaccinated.
Thiotepa may cause liver damage or hepatic veno-occlusive disease. Regular liver function tests are necessary, and any symptoms like jaundice or abdominal pain should be reported to your healthcare provider.
The treatment can impact the nervous system, leading to symptoms such as confusion, seizures, or tremors. Report any neurological symptoms to your healthcare professional immediately.
Long-term use of Thiotepa may increase the risk of developing new cancers. Regular check-ups and screenings are essential for early detection of any malignancies.
Tepadina can harm an unborn baby. Women who are pregnant or planning to become pregnant should avoid this medication. Use effective contraception during treatment and consult your healthcare provider if you are pregnant or planning to conceive.
