Warning & Precautions
Gastrointestinal Adverse Events in Patients with Motility Disorders
Avoid using LOKELMA in patients with severe constipation, bowel obstruction, or impaction, including those with abnormal post-operative bowel motility disorders. LOKELMA has not been studied in these conditions and may be ineffective or worsen gastrointestinal issues.
Edema
Each 5-g dose of LOKELMA contains about 400 mg of sodium, though the extent of absorption is unknown. In clinical trials involving non-dialysis patients, mild to moderate edema was observed, particularly in those taking 15 g once daily. Monitor for edema, especially in patients who need to limit sodium intake or are prone to fluid overload (e.g., heart failure or renal disease). Advise patients to adjust dietary sodium if necessary and increase diuretics as needed.
Hemodialysis Patients
In a clinical trial with chronic hemodialysis patients, most of whom were treated with 5 g to 10 g once daily on non-dialysis days, there was no significant difference in interdialytic weight gain (a measure of fluid retention) between the LOKELMA and placebo groups.
Hypokalemia in Hemodialysis Patients
Patients on hemodialysis may be at risk of hypokalemia during acute illnesses that reduce oral intake or cause diarrhea. Adjust LOKELMA dosage based on potassium levels in these situations.
Diagnostic Tests
LOKELMA has radio-opaque properties and may appear similar to an imaging agent during abdominal X-rays.
