Lupkynis is a calcineurin-inhibitor immunosuppressant prescribed alongside other immunosuppressive therapies for treating active lupus nephritis (LN) in adults.
Recommended Dosage: Start with 23.7 mg twice daily. Take Lupkynis capsules with mycophenolate mofetil (MMF) and corticosteroids. Swallow the capsules whole without crushing, opening, or dividing them. It’s important to take them on an empty stomach, maintaining as close to a 12-hour interval as possible, with at least 8 hours between doses.
If you miss a dose, take it within 4 hours of the scheduled time. If more than 4 hours have passed, skip the missed dose and take the next dose at the regular time. Do not double up on the next dose.
Warning & Precautions
Patients should be monitored for skin changes and advised to minimize sun exposure and avoid artificial UV light (such as sun lamps and tanning beds). They should wear protective clothing and use a broad-spectrum sunscreen with SPF 30 or higher.
Immunosuppressants, including Lupkynis 7.9 mg capsules, increase the risk of bacterial, fungal, viral, and protozoal infections, including opportunistic infections. Regular monitoring for infections is essential. Weigh the benefits and risks for each patient and use the lowest effective dose to maintain the desired response.
Voclosporin, like other calcineurin inhibitors, can cause acute or chronic nephrotoxicity. Regularly evaluate eGFR during treatment and consider dose reduction or interruption if eGFR decreases. Persistent eGFR reduction should be assessed for chronic calcineurin-inhibitor nephrotoxicity.
Hypertension is a common side effect of Lupkynis 7.9 mg capsules and may require antihypertensive treatment. Regularly monitor blood pressure and manage new or worsening hypertension.
Voclosporin may cause a range of neurotoxicities. Monitor for neurological symptoms and consider dose reduction or interruption if neurotoxicity occurs.
Hyperkalemia, which can be severe and require treatment, has been observed with calcineurin inhibitors, including Lupkynis 7.9 mg capsules. Periodically assess serum potassium levels during therapy.
Avoid live attenuated vaccines during treatment with Voclosporin (e.g., BCG, intranasal influenza, mumps, measles, rubella, oral polio, varicella, yellow fever, and TY21a typhoid vaccines). Inactivated vaccines may not be sufficiently immunogenic during Voclosporin therapy.
Female patients should be aware of the potential risk to a fetus and avoid using Voclosporin during pregnancy. They should inform their healthcare provider if they are pregnant or become pregnant. Women should not breastfeed during treatment with Voclosporin and for 7 days after the last dose.
