Zemdri Injection is an aminoglycoside antibiotic used to treat complicated urinary tract infections (cUTI), including pyelonephritis, in patients aged 18 and older. It is effective against the following susceptible bacteria: Klebsiella pneumoniae, Escherichia coli, Enterobacter cloacae, and Proteus mirabilis.
Recommended Dosage: Administer Zemdri at a dose of 15 mg/kg every 24 hours via intravenous (IV) infusion over 30 minutes. This dosage is for patients aged 18 and older with a creatinine clearance (CLcr) of 90 mL/min or higher. The duration of therapy should be based on the severity of the infection and the patient’s clinical response, typically up to 7 days. Dosage adjustments may be necessary based on changes in renal function during treatment.
Warning & Precautions
Nephrotoxicity: Plazomicin may cause nephrotoxicity. It is important to evaluate creatinine clearance (CLcr) before starting therapy and daily during treatment, especially in patients at higher risk, such as those with renal impairment, the elderly, and those on other potentially nephrotoxic medications. If renal function worsens, the benefits of continuing Plazomicin Injection should be reassessed.
Ototoxicity: Zemdri can cause ototoxicity, which may present as hearing loss, tinnitus, or vertigo. This risk is higher in individuals with a family history of hearing loss (excluding age-related hearing loss), those with renal impairment, and those receiving higher doses or prolonged treatment.
Neuromuscular Effects: Aminoglycosides may exacerbate muscle weakness in patients with underlying neuromuscular disorders or delay recovery of neuromuscular function in those receiving neuromuscular blocking agents. Monitor for neuromuscular blockade side effects, particularly in high-risk patients.
Pregnancy Risks: Aminoglycosides, including Zemdri 500 mg, can harm a fetus if administered to a pregnant woman. These drugs cross the placenta, and streptomycin has been associated with irreversible, bilateral congenital deafness in children exposed in utero. Pregnant patients or those who become pregnant while taking Zemdri should be informed of the potential risks to the fetus.
Hypersensitivity Reactions: Severe and sometimes fatal hypersensitivity (anaphylactic) reactions may occur with aminoglycoside antibacterial medications. Before starting Plazomicin, inquire about any previous hypersensitivity reactions to other aminoglycosides.
Clostridium difficile-associated Diarrhea (CDAD): CDAD can occur with nearly all systemic antibacterial medications, ranging from mild diarrhea to fatal colitis. Antibacterial therapy alters the normal flora of the colon, potentially leading to C. difficile overgrowth. If CDAD is suspected or confirmed, discontinue antibacterial medications not directed against C. difficile, manage fluid and electrolyte levels, supplement protein intake, assess antibacterial treatment for C. difficile, and consider surgical evaluation.
Appropriate Use: Prescribing Zemdri without a confirmed or strongly suspected bacterial infection is unlikely to benefit the patient and increases the risk of developing drug-resistant bacteria.
