Galafold is a pharmacological chaperone for alpha-galactosidase A (alpha-Gal A), used to treat adults with Fabry disease who have a specific genetic variant in the alpha-Gal A enzyme that responds to Galafold.
Recommended Dosage: Take 123 mg of Galafold orally every other day at the same time, on an empty stomach. Avoid eating for at least 2 hours before and after taking the capsule, allowing for a total of four hours of fasting. Clear liquids are permitted during this period. Swallow the capsule whole without cutting, crushing, or chewing it. Do not take Galafold on two consecutive days.
Missed Dose: If you miss a dose, take it within 12 hours of the usual time. If more than 12 hours have passed, skip the missed dose and continue with your next scheduled dose as per the every-other-day regimen.
Warning & Precautions
Galafold 123 mg therapy should be initiated and monitored by a healthcare professional experienced in diagnosing and treating Fabry disease. This medication is not intended to be used alongside enzyme replacement therapy.
Monitoring Requirements: Patients starting or switching to migalastat should have their renal function, echocardiographic parameters, and biochemical markers assessed every six months. If clinical deterioration occurs, further evaluation or discontinuation of therapy should be considered.
Proteinuria: No reduction in proteinuria has been observed in patients treated with Galafold 123 mg capsules. This medication is not recommended for patients with severe renal insufficiency, defined as an estimated GFR of less than 30 mL/min/1.73 m².
Concomitant Use: Limited data suggest that taking a single dose of migalastat 123 mg with a standard enzyme replacement therapy infusion may increase exposure to agalsidase by up to five times.
Non-Amenable Mutations: Galafold (Migalastat) is not suitable for patients with non-amenable mutations. The 123 mg capsules are not recommended for children (aged ≥ 12) weighing less than 45 kg.
Pregnancy and Breastfeeding: Migalastat is not recommended for women of childbearing age who are not using contraception. There is limited data on the use of migalastat in pregnant women, and developmental toxicity was observed in rabbits only at maternally toxic doses. Do not use migalastat during pregnancy. Migalastat has been found in the milk of lactating rats, but it is unknown if it is secreted in human milk. Therefore, the potential for drug exposure to a breastfeeding infant cannot be ruled out. Do not use migalastat capsules if you are breastfeeding.
