Onpattro contains a small interfering RNA that targets transthyretin and is used to treat polyneuropathy in adults with hereditary transthyretin-mediated amyloidosis.
Recommended Dosage: Onpattro is available as a liquid in a single-dose vial and must be administered by a healthcare professional via intravenous (IV) infusion. The dosage depends on the patient’s body weight.
- For patients under 100 kg, the recommended dose is 0.3 mg/kg every 3 weeks.
- For patients 100 kg or more, the recommended dose is 30 mg every 3 weeks.
Missed Dose: If a dose is missed, it should be administered as soon as possible.
- If administered within 3 days of the missed dose, continue with the original schedule.
- If more than 3 days have passed, continue dosing every 3 weeks from the new administration date.
Warning & Precautions
Premedication and Monitoring: Patients should be premedicated at least 60 minutes before starting the Onpattro infusion. During the infusion, monitor patients for any signs or symptoms of infusion-related reactions (IRRs). If an IRR occurs, slow down or interrupt the infusion and provide appropriate medical management, such as corticosteroids or other symptomatic treatments.
Handling Infusion Interruptions: If the Onpattro 10 mg infusion is interrupted, consider resuming at a reduced rate only if the signs and symptoms have resolved. For severe or life-threatening IRRs, discontinue the infusion and do not resume.
Vitamin A Levels: Onpattro therapy can reduce serum vitamin A levels. Patients should take vitamin A supplements at the recommended daily allowance. Do not exceed this dosage to normalize serum vitamin A levels, as serum levels do not reflect the total body vitamin A.
Ophthalmologic Monitoring: Patients on Onpattro 2 mg should see an ophthalmologist if they develop symptoms of vitamin A deficiency, such as night blindness.
Pregnancy and Breastfeeding: Patients should inform their healthcare provider if they are pregnant or planning to become pregnant while on Onpattro. Female patients of childbearing age should be aware of the potential risks to the fetus. It is not known if Patisiran passes into breast milk or affects milk production. Patients who are breastfeeding or plan to breastfeed should discuss with their doctor whether Patisiran is suitable for them.
